Short Form Contract:
General Speakers and Performers

To
expedite these types of agreements and
to protect the University contractually,
the University has a short form contract
for guest speakers and performers. The
one page contract contains the many
items to be agreed upon between the
University and the speaker or performer,
including such things as the date of the
engagement, compensation and terms of
payment.
For
these typically small dollar purchases,
departments may download the contract,
fill in the pertinent information and
have the guest speaker or performer
sign. However, the contract is subject
to final review by Procurement Services
and must be signed by a University
procurement officer prior to the
engagement to be valid.
Short
Form Contracts should not be used for
services that exceed $5,000. For
contracts greater than $5,000 total
dollar value a professional services or
non-personal contract should be issued
by Procurement Services. See section on
website Purchasing Policies and
Procedures on professional and
non-personal services contracts.
A
purchase order funding the services must
also accompany the contract.
To
expedite contract issuance and payment,
have the guest speaker or performer also
complete IRS Form W-9 needed to certify
their tax status and submit it with the
short form contract to Procurement
Services.
